Central Drugs also provides patients with the "Patient Care Toolkit", a wealth of resources including contact information, schedules of services, calendar of events, and other resources and activities provided by healthcare providers throughout community.
As medication adherence is critical in optimizing the health of patients, and imperative in decreasing the slope of the cost-curve for the entire healthcare delivery channel, the team at Central Drugs recognizes the importance of medication adherence from a holistic perspective, and focuses on managing patient medication adherence to decrease costs for Payers and to optimize the health of the patients we serve.
Every month Central Drugs staff calls patients who choose to enroll in our “Patient-Centric Adherence Model” of managing refills, to ensure that all medication refills are filled on time. Our staff also inquire as to if patients observe they are ready to have their medications refilled, as well as converse with our patients to about any new questions, comments, or concerns they may have.
Central Drugs calls the doctors when refills are expiring so that:
- Medications will not be interrupted.
- We are immediately aware of any changes in medication regimens.
- If necessary, we can serve as the liaison between the doctor and our patients, often reminding them that they need to visit their doctor.
- If a case manager needs regular communication from Central Drugs, we will provide it.
All that we do achieves multiple goals:
- Decreased stress for patients, families and patient’s healthcare agent(s)
- Higher adherence levels for our patients.
